HOW TO ORDER A TEST

1.  DOWNLOAD THE CHAIN OF CUSTODY

     Download Purity Laboratories' Chain of Custody Form.

 

2.  COMPLETE THE CHAIN OF CUSTODY FORM

     Fill out one Chain of Custody Form (limit 5 samples per form).

3.  SEND THE CHAIN OF CUSTODY FROM WITH THE SAMPLE TO BE TESTED TO:

     PURITY LABORATORIES

     17387 63RD Ave.

     Lake Oswego, OR.

    

4.  Office Hours are 8:30 to 4 p.m. for drop-offs.

5.  Some tests are also available to be ordered on-line.  Click "To Order: in the top menu to place your on-line order.
